What is CDHP?

CDHP, short for Community Disability Housing Plan, is a program designed to support people with disability who require significant ongoing support to live safely within the community.

This program provides access to affordable housing for people with disability. These houses contain accessible features to support the people living in them, enhancing their ability to act independently in comparison to other homes.

Individuals, couples, families, and people who live with carers are all eligible for CDHP, but it does not provide crisis or emergency accommodation.

How do I get CDHP approval?

If you do not have CDHP approval but would like to gain it to use a CDHP home, you can apply for it online or through a local housing office. To have a successful application, you must meet the eligibility criteria and provide supporting evidence. This evidence includes, but is not limited to:

  • Proof of identity.
  • Proof of income and cash assets.
  • Medical reports.
  • Your NDIS plan, if you have one.
  • Specifications of modifications needed in a house.

To apply, you’ll also need to fill out this Request for Housing Form. This form gathers more information about the nature of your circumstances and disability to ensure you are eligible and the right housing can be provided to you.

Where do I send my CDHP application?

When you’ve completed a digital Request for Housing Form, you can submit it online by email to CDHPapplications@dohw.wa.gov.au.

If you need help completing the form or would rather fill out a physical copy in person, you can do it at your local Housing Office. You can find your nearest Housing Office using this online tool.

Is CDHP different to SDA?

Yes. CDHP housing is owned by the WA State Government and distributed to community providers to provide services to.

SDA housing is owned by a registered NDIS provider, and all their dwellings must be enrolled with the NDIA. These properties then choose which providers will provide services to them.

Why do I need CDHP approval?

Many SIL properties supported by Interchange WA are owned and managed by CDHP via a Community Housing Organisation. This means that customers will need CDHP-approved funding in order to live in them.

Does CDHP approval expire?

No. CDHP approval does not expire as time passes. However, you can become ineligible for it if your circumstances change to no longer meet the eligibility criteria.

Can I live in a SIL home without CDHP approval?

Yes. Many SIL homes are not managed by CDHP, meaning you can live in them without CDHP approval. You only need CDHP approval to live in CDHP-owned housing.

Can I live in an SDA home without CDHP approval?

Yes. SDA homes are privately owned by NDIS providers, rather than the WA State Government. You do not need CDHP approval to live in an approved SDA home.

Does Interchange WA provide CDHP housing?

Yes. Interchange WA has multiple CDHP homes.

Once you have CDHP eligibility approval, you will be eligible to apply for housing with a CDHP SIL home. When applying for CDHP housing without a provider, you will be assigned a house based on your preferred living location, split across multiple regions around Perth and Western Australia.
