For young people in foster care, turning 18 often brings uncertainty about their future living arrangements. One dedicated foster care mum wanted to ensure that the young person in her care had continued support beyond the foster system. To explore long-term accommodation options, her support coordinator introduced her to Interchange and their NDIS Host Services through Individualised Living Options (ILO).

Exploring the Possibility of Becoming a Host

Interchange guided her through the ILO exploration process, assigning a dedicated contact person to assist every step of the way. Over several months, the team worked closely with therapists, support coordinators, and other key individuals to design a tailored Host Arrangement that would provide ongoing stability and support. With a thorough application process and close collaboration with the support coordinator in submitting a strong case to the  NDIS, funding approval was successfully secured.

Now, as an official NDIS Host with Interchange, she continues to provide a nurturing home environment, helping the young person in her care transition smoothly into adulthood.

What is a Host Arrangement?

A Host Arrangement is a welcoming accommodation option where a Host invites a person with a disability to share their home and be a part of their life. It provides live-in support, companionship, and a safe, stable environment.

Each Host Arrangement is tailored to the needs of both the Host and the person they support—whether it’s full-time or short  stays. While Hosts play a central role in providing care and connection, additional rostered Support Workers can also assist with daily activities and community participation.
